New Hamphsire CoP Survey (DOC). In 2009 New Hampshire developed this survey to be used by all of their NH Community of Practices.The NH Transition Community of Practice Coordinating Group completes the survey in the Fall. The NH Community of Mental Health completes this survey in the winter, and The NH Transition Community of Practice (local COPs) complete this survey now in the Spring (question 28 is only completed by this group). NH developed this word version of the Survey to use as a tool with our Local COPs to make sure they are know the components of a COP and reminding their groups of this strategy.
